If you are eligible for another year of CCHD funding, you will receive an e-mail with access to the interim report and online application the week of November 22. The deadline for submission of the online interim report and application for an additional year of funding is December 17 (11:59 P.M. Eastern Time).

If you are not eligible for another year of funding, the Interim Report Form is due February 15. Access the Interim Report Form using the links below.

Final Reports

In doing our part in sustaining the Earth, we have enabled our online application site to now accept all required reports and documentation via the internet.  Year End Reports and Year End Expense forms for the Community Organizing and the Economic Development Program can all be uploaded through the CCHD application website.  Simply go to www.grantrequest.com/SID_464/Default.asp to login to your account.  Once you have logged in, click on the "Requirements" tab and follow the instructions to upload documents from your desktop. Pay special attention to due dates, so as not to upload your documents under the wrong grant year. For example, final reports for the 2008-2009 grant year's due date should be in 2009. 

Year end reports are due no later than August 3. Access the year end report forms below.
